Submit this form to confirm the following:

• OPTION 1: Attend in-person and participate only in certain dance sections
  
• OPTION 2: Attend in person (no dancing) and submit a video for coaches to review
• OPTION 3: Transfer to the At-Home Pass and submit a video for coaches to review
• OPTION 4: Cancel your pass

Please note the following:

• All Terms & Conditions agreed to at purchase will apply.
• When OPTION 3 or 4 is submitted on this form, the associated dancer portals will be removed from your account. If transferring to the At-Home Pass, the At-Home Dancer Portal will be applied to your account.
