REFUND REQUEST FORM 

Please note the following:

• Allow thirty (30) business days for refund requests to process.
• When filling out the form below, please input the dancer's full name & the email that was used to originally purchase the event pass.
• All Terms & Conditions agreed to at purchase will apply.
• Upon submitting this form, the associated dancer portals will be removed from your account. If transferring to At-Home Pass, the At-Home Dancer Portal will be applied to your account. 
• Please ensure your decision is final, as refunds and transfers cannot be reversed once processed.
